TYPES OF DATA COLLECTED
Applied Medical receives and processes personal information from our Customer (your Hospital or Residency Program, “Hospital”) about their faculty, residents, and fellows, as needed to provide our products and services to Hospital, meet our legal obligations, and conduct our business operations. We use the following personal information to provide the Simsei Implementation Solution, including the provision of training curriculum, provision of technical support, access to the broader Simsei community, and other services under the Agreement between Applied Medical and Hospital, and customer relationship management:
- Business contact details: Name, email, PGY, title, specialty, phone number, other contact details
- Educational data: Assessment scores, course progress, course scheduling, course attendance, video assessments
- IT systems information (i.e., user ID, password, asset IDs, and filename extensions)
- Content you submit: your posts or comments within the Simsei Implementation Solution and to the Simsei community. Your content may include text, audio, or videos.
USE OF YOUR INFORMATION
As a data processor, Applied Medical will collect and process your personal information only as instructed or permitted by Hospital or you. Applied Medical maintains reasonable physical, technical, and organizational safeguards and security controls to protect your personal information and avoid unauthorized access or disclosure.
Applied Medical will disclose your personal information to Hospital and to other entities when instructed by Hospital or you. When you submit content (including a post, comment, or video) to the broader Simsei community, anyone within the Simsei community, including users outside of your Hospital or Residency Program, will be able to see that content, the name associated with that content, and the date and time you originally submitted the content. We may also disclose your personal information to our affiliates, service providers, and third party processors as needed to provide the products and services that you and Hospital have requested and in the ordinary course of our business operations. These entities are contractually bound to limit the use of your personal data as needed to perform the services. We will also disclose personal data when required to do so by law, such as in response to a subpoena, including to law enforcement agencies and courts in the United States and other countries where we operate. Applied Medical will not otherwise sell, rent, publish, or exchange your personal information to any other organization or entity unless you are first notified and expressly agree to it, or as otherwise required by law.
Applied Medical may use aggregated, non-identifying, electronic data collected from the use of our products and services to operate, analyze, improve, and develop the Simsei Implementation Solution.
DURATION OF PROCESSING
Applied Medical retains personal information for as long as necessary to provide the products and services to Hospital, comply with our legal obligations, and enforce our Agreement with Hospital. Hospital is the data controller and defines the retention policy of the personal information processed subject to the Agreement between Applied Medical and Hospital. Applied Medical will delete personal information from the Simsei Implementation Solution and its systems and servers when no longer needed or within three (3) months upon termination the Agreement between Applied Medical and Hospital.

YOUR RIGHTS WITH RESPECT TO YOUR INFORMATION
Applied Medical will support and assist Hospital in addressing your data privacy requests, in accordance with the terms of the Agreement between Applied Medical and Hospital. You may also access, delete, or correct certain personal information by signing in and directly updating your account information in the Simsei Implementation Solution.
